Trichrome Vitiligo: What the Tan Border Around a Patch Means
Most vitiligo patches have a fairly clean border — depigmented skin transitioning fairly directly into normally pigmented skin. Trichrome vitiligo looks different, and if you’ve noticed a tan or light brown band between a white patch and your normal skin tone, there’s a specific term and explanation for it.
What “trichrome” actually describes
Trichrome vitiligo gets its name from the three visible zones of color in a single patch: the fully depigmented white center, an intermediate band of tan or light brown color, and the surrounding normally pigmented skin at its usual tone. Instead of a sharp transition from white to normal, there’s a visible gradient step in between.
What’s happening biologically
The intermediate tan zone reflects a gradient of partial melanocyte loss, rather than the complete absence of melanocytes seen in the fully white center. Some melanocyte activity is still present in that band — reduced compared to normal skin, but not entirely gone the way it is in the depigmented center.
Some research has suggested this intermediate zone may represent an area of active disease progression — melanocytes in the process of being lost, captured mid-transition, rather than an area that has stabilized. This isn’t a universally settled finding and shouldn’t be treated as a certain prognosis, but it’s a reasonable part of why dermatologists pay attention when trichrome patterns are present.
Does this change anything about treatment or outlook?
Trichrome is a description of appearance, not a separate condition requiring different treatment. The same evidence-based treatments — NB-UVB, topical calcineurin inhibitors, Opzelura, and others — apply the same way they would to any other vitiligo presentation.
What it may be useful for is a more informed conversation with your dermatologist about disease activity specifically in that area. If the intermediate zone is present, it’s worth asking directly whether they consider that patch actively progressing or stable, since that can shape how proactively you and your dermatologist approach treatment timing for that specific area.
What happens to the tan zone over time
There isn’t one fixed path. The intermediate zone can remain stable for an extended period without further change, it can gradually convert to fully depigmented skin over time, or — with treatment — it can repigment along with the rest of the patch, the same way any actively treated vitiligo area might respond. Individual variation here is genuinely wide, and there’s no way to predict which direction a specific patch will go without watching it over time.

Noticing a trichrome pattern is a good reason to mention it specifically to your dermatologist rather than assuming it’s just a cosmetic detail — the possible connection to active progression means it’s worth factoring into the broader conversation about your disease activity and treatment timing. It’s not, on its own, cause for alarm — just a detail worth naming clearly when you’re describing your patches.
